The healthcare system is designed for the healthcare system, not the people who use it. To change this paradigm, professionals working in the area of case management and care coordination need to understand the real-world problems that impact effective care coordination. This session discusses care coordination and changes that need to be done to ensure the process is patient-centered if we are to improve the delivery of care.
